Work Permit Instructions
Instructions for the ES
All
students from the ages of 14 through 17 are required to have a Work Permit
before they begin work, unless they are in the entertainment business, in which
case they will need an Entertainment Permit, see last entry for information.
ES Approval is required to obtain a regular Work Permit or an Entertainment
Permit. Approval is granted based on the
student’s satisfactory academic progress, and can be revoked at any time if the
student’s schoolwork is not acceptable. Please note, only the school office can
sign a permit of any kind (Work Permit or Entertainment Permit).
Work Permit Procedures:
1.
Work Permit
Checklist & Statement
of Intent to Employ Minor and Request for Work Permit : Student obtains
both from school website.
2.
For Minor to Complete section of the Statement of Intent to
Employ Minor and Request for Work Permit: Completed by student.
3.
For Employer to Complete: Employer completes, providing sufficient
detail under “Minor’s Work Duties” i.e. description of job responsibilities,
tasks, duties to be performed by the student, etc. Employer also
completes the maximum number of hours that the minor will be working and signs
form.
4.
For Parent or Guardian to Complete section: Parent completes, signing and
dating where noted.
7.
Copy of the
student’s Social Security Card with the same name as the one in which the work
permit is requested must be included with application.
8.
Copy of the student’s official Birth
Certificate must be
included with application, unless already in Webfiles. (here is the link to the CA State Vital Records, if needed)
9. Work Permit ES Approval/Denial Template: ES emails to
the office, giving Approval or Denial for student to obtain a permit.
For 14 or 15
Year Old Students Only: ES
emails confirmation to the office that student is earning Career Exploration
credits.
10. Office Receipt of All of the Above: Work Permit is completed and mailed to
student within 3 days of receiving the complete packet.
11. Student
receives Work Permit,
signs, and gives to employer.
12. Renewing
a Work Permit: ES emails
the office the date the initial work permit was granted, following the
procedures above. However, No copies of the
Social Security Card or Birth Certificate are required—the office received them
when processing the initial work permit.

Entertainment Permit Procedures
1. Application
for Permission to Work in the Entertainment Industry (Entertainment Permit application): Available
to parents through this link, in addition to other pertinent information http://www.dir.ca.gov/dlse/DLSE-Forms-CL.htm
2. Top
of form: Parent completes,
signs, then mails (no faxes accepted) form to school office, Attn: Work
Permits.
3. Work Permit ES Approval/Denial Template: ES emails to
the office, giving Approval or Denial for student to obtain a permit.
4. School
Record portion: School
office completes.
5. Office mails application to parent.
6. Parent
mails (no faxes accepted)
or takes in completed application to any office of the Division of Labor
Standards Enforcement, including a self-addressed stamped envelope.
