Examples:
Driver's Education
XXX is taking this class as a CP
activity. XXX is taking a driver's ed class once a
week. For this week she completed a worksheet which
posed different traffic problems. Her
answers reflected what she would do in certain situations. One of these situations was to list all
possible solutions to the traffic problem caused by traffic lights being
out. She also completed a pre-test on
her knowledge of traffic signs and signals.
Computer Skills 1A
XXX is taking this class as a CP
activity. XXX is taking a computer class once a week. For their first class
she completed a pre-test of her computer skills and prior knowledge. She took a typing test where she had to
complete a minimum of 30 wpm; she typed 60 wpm.

NOT DUPLICATE content every learning period with no new learning stated, not

LRs
cannot be created ahead of time. New learning cannot
be documented until you know what actually happened.

WHAT
did the student learn? - New and possible continued concepts learned need to be
stated every learning period
Examples:
Algebra1A:
XXX recalled and studied how to find solutions
to sets of open sentences. She studied examples and solutions, defined and practiced to recognize Commutative, Associative,
and Distributive properties. She completed exercises to represent numerical
relationships stated in words by mathematical expressions or equations. She
completed Ch.1 test.
English
Unit 3 - XXX studied spelling concepts with Latin roots
such as manu, anim, arbitr, art, cap, capt and various forms of nouns with plurals. She further studied and practiced using noun and pronouns. She looked up the meanings in the dictionary
of the newly learned spelling words. XXX
worked on a word search using the 25 word spelling
list related to the Latin roots. She
replaced common noun with proper nouns within sentences. She wrote sentences using her spelling words
and the various different types of nouns.
She found and underlined linking verbs, and predicate nominative. She took the end of unit test and received an
86%.

HOW
did the student learn the concepts? Needs to be stated in
EVERY subject area.
Example:

Student
is working on 1 subject area for one learning record
period.
If a student and the family are
working for example on Reading only for a 20 day period you will need to state
in the other subject areas “Student did not work in
this area during this learning period.” Remember
as well that the amount of content you are entering should justify the extent
of the amount of work in that subject area for those 20 days. It should be much lengthier since this is the
only area that the student worked on for 20 days.

DO YOU KNOW they learned it? - Assessment statement required for each
subject area.
K-8 needs to be
documented throughout the semester
9 – 12 mandatory in every subject area and every learning period throughout the
semester
Examples:
English 9A
This month XXX worked
on different spellings related to Greek roots such as cosm,
derm, ethn, geo, and graph
and gram. She formed the plural of nouns
for grammar activities. She practiced
writing technical directions. She
learned about consumer documents and how to understand their meanings
effectively. She answered questions with fill in, changed spellings for singular into plural words in adding
s and es.
She looked up definitions in the dictionary for various spelling
words. She read a technical document
involving an Emerson portable CD player.
She then had to practice writing technical directions for a blender
appliance in the kitchen. She read
various warranty pieces and studied what they included. She matched spelling words to other synonym
words. At the end of this unit she took the unit
test. She received a 100% on this test.
XXX identified the
names of the letters in the alphabet. She said a word that begins with each
letter. She sang to me the ABC song.
She listened to stories that were read to her.
Writing: XXX practiced writing capital and lowercase letters. She wrote the
letters A, B, C, D, E, F, G, H, K, L, M, N, O, Q, R, V and X. She wrote the
days of the week and the name of the month. She orally recited to me the days of the week, month, date and year.
She completed worksheets on rhyming words.

Student
dances (should not be present tense but
past tense) to improve muscle tone and maintain fitness.
Good example of same content listed above:
P.E.
Student danced to improve muscle tone
and maintain fitness. During her exercise routine XXX worked on lower body muscles. XXX watched a video dance routine. Specific dance routines included, “the twist,
MTV inspired dance routines, and the Macarena”.
XXX occasionally danced with her friends and also
XXX attended a “dance night” with a friend at the public high school.

Always spellcheck your content for each learning period
·
Truancies and subject areas not complete for learning period.
State if a student is
truant during that learning period in the content area. If a student did not work on a particular
subject during that learning period state that
“Student did not work in this area during this learning period.”
·
Standards and the Standards Box
Standards must be checked
off every learning record period.
Students should be progressing in all of the standards for their grade
level throughout the year. If a student has the same specific standard checked every learning period
for the whole year this is not acceptable.
Students should be working towards all of the standards at their grade
level. If a student for several learning
periods is working on the same standard you may check
it again but new learning should be taking place. This new learning will be
reflected in the new specific standards that you check. At the end of the year
your student should have a year-long list of all of the standards that the
student has worked on. This includes
PE. It is not acceptable to have the
same standard checked for the entire year because the student worked on an
activity log. Think about other
activities (for example researching a sport, game, and famous sports person for
that standard that is required) that will meet the requirements for the other
standards that are listed.
