Immunization Process for continuing 6th grade students in IEM Schools
This process is for all currently enrolled 6th grade students in IEM schools. 6th grade enrollment applications received after the Spring cut-off enrollment date will be treated as a 7th grade enrollment application and put into “pending” status until all enrollment info is received.
1. In early January of the 6th grade year, all 6th grade parents will be mailed the 7th grade immunization requirements letter, a health card, and a letter explaining our process and timeline for information, March 1.
2. On March 1st, a 2nd request letter will be mailed out to those who have not responded to the January request. This letter will give them 30 days to submit the necessary immunization information.
3. 6th grade student enrolling between the date of the January letter and March will be mailed the first letter as soon as they are activated.
4. On April 1, the ES of each 6th grade student will be notified electronically (Webfile alert or email; format will be agreed to by IEM departments annually in late March) which of their students have not yet complied with the immunization requests. Specific 7th grade immunization requirements will also be made available to the ESs with the alert.
5. When completing the Fall Student Agreement for each 6th grade student on the alert list, a health card containing the required immunization information or a signed waiver must also be completed and mailed with the SA to Student Records by the ES.
6. A continuing Student Agreement arriving in the IEM office for a 7th grade immunization alert student will be returned to the ES, with a form letter attached explaining that it cannot be accepted without the required immunization information checked on the form.
7. Any 6th grade parent on the immunizations alert list who will not complete
a health card along with a SA when requested at their in person meeting will
be informed by the ES that they may not enroll for 7th grade with an IEM school.
The ES then does not submit the SA, but instead submits a drop form for that
student effective the last day of the school year.
